Definitely worth it to add the tunnels!

This is a must see when visiting Barcelona. I also highly recommend adding the towers part of the tour, which allows you to ride an elevator to the top of one of the towers for great, close up views of the basilica and the city. It's 400 steps down, so probably not for those who are challenged by mobility. Our tour guide was interesting and engaging. I would recommend this tour and tour guide.
